After their 40-9 defeat to England, Wales were starring down the barrel of a third defeat in the RBS 6 Nations when Italy took a 17-13 lead in halftime.
But they got themselves out of trouble with tries from Luke Morgan and Baker as he intercepted a pass from the unfortunate John Apperley with just five minutes remaining.
Coach Danny Wilson made seven changes from the side that suffered a chastening defeat to England but they made a terrible start as Vittorio Marazzi made a strong run from the back of a scrum and Guido Calabrese took full advantage for the opening score.
Michael Odiete converted before Apperley added a penalty to give the Italians a 10-0 cushion but Wales responded on 12 minutes when prop Rob Evans barrelled his way over after a barnstorming run. Sam Davies added the extras and then slotted a penalty to level up the scores.
But Italy were soon back in front thanks to a solo try from winger Leonardo Sarto who cut his way through the Wales defence with a jinking run.
A second Davies penalty ensured Wales only trailed by four points at the break with the two fly-halves trading further three pointers early in the second half.
It was not until the 60th minute that Wales led for the first time in the match when 17-year-old Jack Dixon broke clear before offloading superbly into the path of the onrushing Morgan. Jordan Williams converted to put Wales 23-20 ahead.
But parity was restored by Apperley on 67 minutes before Italy's hero turned zero when his pass had too much air on it and Baker, who was named man of the match, needed no second invitation to latch onto the ball and race the length of the field for the match-winning score.
